Hyperpigmentation, Explained: What's Actually Happening Under Your Skin (And What Actually Helps)

If you've ever stood in front of a skincare aisle wondering why one "brightening" serum costs three times as much as another, or why your dark marks seem to fade so slowly no matter what you try, this one's for you. Hyperpigmentation is one of the common concerns I see in clinic here in Didsbury, and also one of the most misunderstood. So let's actually unpack what it is, why it happens, and what genuinely helps, without the marketing noise.


What is hyperpigmentation, really?


Melanin is the pigment that gives your skin its natural colour, and it's there to protect you — it's part of how your skin defends itself against UV radiation. The problem starts when your skin produces too much of it in specific areas, leaving behind darker patches or marks. That's hyperpigmentation.

It doesn't have one single cause. In clinic, the drivers I see most often are:


  1. Sun exposure - by far the most common trigger

  2. High-energy visible light (from the sun, not your phone screen) - which can also activate pigment in certain skin types

  3. Inflammation - a spot, a scratch, an irritated patch of skin can all leave pigment

    behind once it heals

  4. Hormones - particularly relevant in pregnancy, with certain medications, and with

    some hormonal conditions



The three faces of hyperpigmentation


Not all dark marks are the same, and they don't all respond to treatment in the same way:


  • Age spots - patches that build up gradually over years of cumulative sun exposure, typically appearing later in life.

  • Post-inflammatory hyperpigmentation (PIH) - the brown marks left behind after a spot, injury, or irritation. If you have fairer skin, you might notice red marks instead, which is a related but slightly different process.

  • Melasma - patches that tend to appear earlier, often between 20 and 40, and are strongly linked to hormones. Pregnancy, contraceptive medication, and certain other hormonal factors are common triggers, alongside sun exposure and visible light.


Knowing which one you're dealing with matters, because it changes what's actually worth trying.


Why "brightening" isn't a single mechanism


Here's the part most skincare marketing skips entirely:

Melanin production happens in stages & different ingredients interrupt it at different points.


  • Before melanin is made - some ingredients work upstream, regulating the processes that trigger pigment production in the first place. Retinoids fall into this category, alongside ingredients like ceramides and sphingosines.

  • During melanin production - this is where most of the well-known "brightening" ingredients work, by inhibiting an enzyme called tyrosinase, which your skin needs to actually produce melanin. Hydroquinone, arbutin, kojic acid, azelaic acid, vitamin C, resveratrol, and zinc all act at this stage.

  • After melanin has already been made - a final group of ingredients works by limiting how much of that pigment spreads to surrounding skin cells. Niacinamide is the best-known example here, alongside ingredients like liquorice extract and vitamin E.


The honest takeaway:

Very few single ingredients cover all three stages. That's exactly why an effective approach almost always combines more than one ingredient, rather than relying on one "hero" product to do everything.



What the evidence actually shows


A few standout findings worth knowing:


  1. Niacinamide can meaningfully reduce how much pigment spreads to surrounding skin cells, and its overall lightening effect has been compared favourably to hydroquinone in research.

  2. Azelaic Acid is selective - it targets abnormal pigment (like sun damage or inflammation-related marks) while leaving normal pigment largely alone, and has performed comparably to hydroquinone in studies, particularly combined with glycolic acid.

  3. Vitamin C in its most active form (ascorbic acid), has shown effectiveness comparable to hydroquinone for lightening skin and dark spots.

  4. Resveratrol the antioxidant found in grapes is often overlooked, but research suggests it may outperform hydroquinone, arbutin, and vitamin C individually, and works particularly well paired with glycolic acid.


None of this means "more ingredients is automatically better." It means the right combination, chosen for your specific type of pigmentation, matters more than any single product.


The gold-standard comparison - and why it's not the full picture


The most well-established prescription approach for stubborn hyperpigmentation combines three different medications, each working at a different stage of melanin production.

It's effective - but it isn't without downsides, including skin thinning, changes in pigmentation, visible blood vessels, and it's unsuitable during pregnancy.

The good news is that many over-the-counter ingredients can achieve a similar effect, stage by stage, without the same risk profile - which is exactly why a personalised combination, rather than a single product, tends to work best.


Patience is part of the plan


One of the most common questions I get is "how long will this actually take?"


Realistically, most brightening ingredients take weeks to months to show meaningful change, and it varies quite a bit by ingredient - some acting within a matter of weeks, others needing several months of consistent use before results become clear.


If a product promises overnight results, that's worth being sceptical of.


The step people skip


Whatever combination of ingredients you're using, none of it matters much without consistent sun protection. UV radiation is one of the main triggers of melanin production in the first place, and for certain skin conditions, visible light matters too - which is why some sunscreens include iron oxide for broader protection. Reapplication throughout the day, avoiding peak sun hours, and simply not picking at inflamed skin all make a real difference alongside any product or in-clinic treatment.
